Job Summary
The Transplant RN Care Coordinator is responsible and accountable for clinical expertise for transplant patient populations. The Transplant RN Care Coordinator functions as a clinician, educator, investigator, consultant, and leader. Clinical judgement is based on a synthesis of current research, concepts, principles, theories, and outcome to solve complex problems. The Transplant RN Care Coordinator functions as an integral part of the transplant interdisciplinary team, collaborating with the nurse coordinators, transplant physicians and in accordance with established transplant policies and procedures and contributes to excellence in patient care, research, and teaching within the transplant center.
Job Responsibilities
Provides specialized nursing treatment, care, and education to patients in accordance with physician/nursing orders and established policies and procedures.
Serves as a resource to patients and families, providing preoperative counseling and follow-up problem solving after discharge from the hospital.
Coordinates the development, utilization, and evaluation of appropriate educational resources and interventions that assist the patient/caregiver in developing the skills for self-care.
Promotes evidence-based practice through staff education and participation in developing, implementing, and evaluating process standards such as plans of care, protocols, and guidelines. As an APS member, collaborates in preparing standardized orders (CPOE) and protocol-driven practice updates and reviews.
Assesses the educational needs of the patient/caregiver as it relates to the disease process, alterations in function, and assimilation back into the home and community.
Utilize current nursing and clinical research to support evidence-based clinical decision-making and practice
Participates in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies and/or clinical research.
Utilizes teaching and learning theories to assist patients and families with physical and emotional impact of body changes and chronic illness.
Provides clinical experience in a preceptor role for nursing students attending institutions that have contractual agreements with DUHS.
Participates in the orientation of new nursing staff and provides expert coaching and guidance through both formal and informal one-on-one teaching with nursing staff.
Work Schedule
This is a full-time salaried position that does NOT require working rotational weekends, holidays or call. This position does extensive patient assessment in the clinic environment, phone triage, and patient education to ensure exemplary care.
Shifts:
Day Shift, Monday Friday, hours determined per program practice
Minimum Qualifications to Apply
BSN Degree
A minimum of 3 years RN bedside experience is required. Transplant or chronic disease management nursing experience is highly preferred.
Current RN license issued by NCBON or multi-state compact RN licensure.
BLS required

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
